- Hormone and endocrine panels — testosterone (total and free), estradiol, progesterone, DHEA-S, cortisol (AM/PM), FSH, LH, prolactin, and thyroid panels (TSH, free T3, free T4, reverse T3). Relevant for fertility evaluation, perimenopause/menopause, hypogonadism, adrenal function, and polycystic ovary syndrome.
- Thyroid antibody testing — anti-TPO and anti-thyroglobulin antibodies for autoimmune thyroid disease (Hashimoto's thyroiditis, Graves' disease); often ordered when TSH is abnormal or thyroid symptoms persist despite normal TSH.
- Nutrient and micronutrient panels — vitamin D (25-OH), vitamin B12, folate, ferritin, magnesium, zinc, copper, and selenium. Useful for fatigue workup, hair loss, immune dysfunction, and vegetarian or restricted-diet monitoring.
- Autoimmune and rheumatologic panels — ANA (antinuclear antibody), anti-dsDNA, RF (rheumatoid factor), anti-CCP, complement C3/C4, HLA-B27. Ordered when symptoms suggest lupus, rheumatoid arthritis, ankylosing spondylitis, or other autoimmune conditions.
- Cardiovascular risk panels — Lp(a) (lipoprotein a), apolipoprotein B, hs-CRP (high-sensitivity C-reactive protein), homocysteine, and advanced lipid fractionation (LDL particle number, sdLDL). These go beyond a standard lipid panel to assess residual cardiovascular risk.
- Infectious disease and STI testing — HIV Ag/Ab combo (4th generation), hepatitis B surface antigen, hepatitis C antibody + reflex RNA, syphilis (RPR or treponemal), HSV-1/HSV-2 IgG, gonorrhea/chlamydia NAAT (urine or swab), and Lyme disease serology.
- Tumor markers — PSA (prostate), CA-125 (ovarian), CEA (colorectal monitoring), AFP (liver, germ cell), CA 19-9 (pancreatic). Typically used for monitoring known cancers or in high-risk screening contexts — not as general cancer screening in low-risk patients.
- Pharmacogenomic testing — DNA-based testing for genetic variants affecting drug metabolism (CYP2D6, CYP2C19, CYP2C9, TPMT), relevant for prescribing antidepressants, antipsychotics, anticoagulants, and pain medications.
Which specialty tests require a mobile phlebotomist?
Not all specialty tests can be collected at home equally well. Mobile phlebotomy is ideal for the majority of blood-based specialty panels, but a few have specific collection requirements:
- Time-sensitive draws: Cortisol levels are time-sensitive — AM cortisol must be collected between 7–9 AM; PM cortisol between 3–5 PM. A mobile phlebotomist who arrives at the scheduled time can collect these accurately. Fasting insulin and glucose require an 8-hour fast, which is easily maintained at home.
- Cold-chain requirements: Some specialty markers (cryoglobulins, certain hormone assays) require the specimen to be kept at body temperature immediately after collection or to be centrifuged within a specific window. A trained mobile phlebotomist manages this with proper transport media; confirm these requirements when booking.
- Non-blood specimens: Urine-based tests (24-hour urine cortisol, urine catecholamines, urine protein), stool tests (calprotectin, H. pylori antigen, microbiome analysis), and saliva panels (salivary cortisol rhythm, DHEA) are self-collected — no phlebotomist required. These are typically mailed directly to the lab using a collection kit.
- Genetic and pharmacogenomic tests: Usually collected via buccal swab (cheek swab) — self-collected and mailed. No blood draw needed.
Ordering specialty tests: physician-ordered vs. direct-access testing
Specialty tests can be accessed two ways:
Physician-ordered: Your doctor orders the test, typically through your insurance. The lab bills insurance; you pay your plan's cost-sharing. Physician-ordered testing ensures results are interpreted in clinical context and appropriate follow-up is arranged.
Direct-access (patient-directed) testing: In most U.S. states, patients can order certain lab tests without a physician's order. You pay out-of-pocket (prices vary widely by panel), and results are returned directly to you. This approach is common for hormone panels, nutrient levels, STI testing, and cardiovascular risk markers. If a result is abnormal, you will need to follow up with a physician for interpretation and next steps.

Key questions to ask before ordering a specialty test
Before ordering any specialty test — particularly from a direct-to-consumer testing company — ask:
- "Is this lab CLIA-certified?" Clinical Laboratory Improvement Amendments certification is required for any lab that processes clinical specimens in the U.S. Results from non-CLIA labs are not valid for medical diagnosis.
- "Will my physician receive the results?" Some specialty tests report only to the patient, bypassing the physician. For tests that will inform treatment decisions, results should route to your provider.
- "What type of collection does this require?" If the test requires a venous draw but is marketed as an "at-home test," you will need a certified phlebotomist to collect the sample — the "at-home" part refers to the convenience of not going to a patient service center, not to the collection technique.
- "What is the reference lab?" For clinically actionable results, confirm the testing laboratory and its accreditation status (CLIA, CAP, or COLA).
