Lithium has one of the narrowest therapeutic windows in psychiatry. The difference between a therapeutic trough of 0.8 mEq/L and a toxic level of 1.5 mEq/L can be the difference between stable mood and kidney damage, tremor, or encephalopathy. Lithium trough levels must be drawn consistently at 12 hours post-dose, plus or minus 30 minutes, to be interpretable. A level drawn at 8 or 16 hours post-dose produces a number that cannot be accurately compared to standard reference ranges designed for 12-hour sampling. Clozapine is one of the most effective antipsychotics available for treatment-resistant schizophrenia but carries a risk of agranulocytosis in approximately 1 to 2 percent of patients. The FDA's REMS program requires mandatory absolute neutrophil count monitoring before every clozapine prescription refill: weekly for the first 6 months, biweekly for the second 6 months, and monthly thereafter for stable patients. Patients whose ANC falls below defined thresholds must interrupt therapy immediately.
