If you have ever watched a phlebotomist fill several different colored tubes during one visit, you have seen the tube color-coding system in action. Each cap color represents a specific additive designed to prepare the blood sample for a particular category of testing — the color itself is a shorthand the entire lab industry uses to avoid mix-ups, standardized under CLSI collection guidelines that reference labs, hospitals, and mobile phlebotomy services all follow.
Why tubes are color-coded
Different tests require the blood to be handled differently once it leaves the vein. Some need an anticoagulant to keep the sample liquid for cell counting or plasma testing; others need the blood to clot normally so serum can be separated. The color coding, standardized across manufacturers like BD and Greiner, tells the lab team exactly which additive is inside without needing to check a label first — and tells the phlebotomist which tube to fill, and in what order, to avoid contaminating results with additive carryover.
Common tube colors and what they're used for
- Lavender (purple) top — EDTA: Used for a Complete Blood Count (CBC) and other hematology tests. EDTA binds calcium to prevent clotting while preserving the shape of blood cells for accurate counting under a microscope or automated analyzer. - Green top — heparin: Often used for potassium, and more broadly for chemistry panels sensitive to clotting artifacts. Heparin prevents clotting without interfering with many chemistry results, which is why it's frequently chosen when accurate potassium levels matter — clotting can rupture red blood cells (hemolysis) and falsely elevate a potassium reading drawn in a tube that isn't appropriate for it.
- Red or gold/tiger top — clot activator / serum separator: Used for most standard chemistry panels, lipid panels, and many hormone tests. These tubes allow the blood to clot normally so serum can be separated for testing, typically by centrifuge.
- Light blue top — sodium citrate: Used for coagulation studies like PT/INR and PTT. Sodium citrate binds calcium in a precise 9:1 blood-to-additive ratio needed for accurate clotting-time measurement — this tube must be filled completely, or the ratio is thrown off and the result can be invalid.
- Gray top — sodium fluoride / potassium oxalate: Used for glucose testing, since the additive inhibits ongoing cellular metabolism (glycolysis) that would otherwise falsely lower a glucose reading the longer the sample sits before processing.
- Yellow top (SPS) or ACD: Used for blood cultures or certain specialty and genetic testing that requires preserving cells in a specific state.
Why order and tube choice matter
Filling tubes in the correct sequence — the order of draw — prevents additive carryover between tubes, which can contaminate results. The standard CLSI sequence runs roughly: blood culture bottles first (if ordered), then light blue (citrate), then red/gold (serum), then green (heparin), then lavender (EDTA), then gray (fluoride). You don't need to memorize tube colors to have a draw done correctly — that is the phlebotomist's job. But understanding the system explains why a single visit sometimes involves several tubes even for what feels like “one blood test”: your provider may have ordered a panel that spans multiple tube types (for example, a CBC plus a chemistry panel plus a coagulation study), each requiring its own tube and additive.
What happens if the wrong tube is used for a test
Using the wrong tube additive for a given test is one of the most common reasons a lab rejects a sample and requests a redraw. A few examples of what can go wrong:
- Drawing a potassium test into an EDTA (lavender) tube instead of the correct tube can produce a falsely high potassium result, because EDTA itself contains potassium salts in some formulations and can also cause cell changes that skew the reading.
- Drawing a coagulation study (PT/INR, PTT) into a partially filled light blue (citrate) tube throws off the precise blood-to-additive ratio required, which can produce an inaccurate clotting time.
- Drawing a CBC into a tube without EDTA can allow the sample to clot, making an accurate cell count impossible.
