Allergen-specific IgE blood testing measures the concentration of IgE antibodies directed against individual allergens including foods like peanut, tree nuts, shellfish, milk, and wheat; inhalant allergens like grass pollens, dust mites, cat dander, and mold; insect venoms; and drug allergens. Unlike skin prick testing, specific IgE testing does not require stopping antihistamines, avoids systemic reaction risk, and can be performed on patients with severe eczema or dermographism who cannot undergo skin testing. Sensitivity and specificity vary by allergen. Your allergist interprets results in context of clinical history, not as standalone positives or negatives disconnected from your symptom pattern.
Understanding IgE levels, component testing, and clinical thresholds
A positive specific IgE result does not automatically confirm clinical allergy. Sensitization, the presence of IgE antibodies, is common without clinical reactivity. Your allergist uses the level, pattern across related allergens, and your clinical history together. Component-resolved diagnostics go further, identifying which specific molecular components of an allergen you are sensitized to. For peanut, Ara h 2 sensitization predicts higher risk of systemic reactions than Ara h 8, which is often cross-reactive with birch pollen.
