HDL facilitates reverse cholesterol transport from tissues back to the liver for clearance. Triglycerides reflect dietary fat, carbohydrate intake, insulin resistance, and metabolic syndrome risk. Non-HDL cholesterol captures all atherogenic particles and outperforms calculated LDL-C in certain populations. Fasting is required for accurate triglycerides and calculated LDL-C. A meal within 8 to 12 hours before a draw can elevate triglycerides by 50 to 100 mg/dL or more and correspondingly distort the calculated LDL. Some direct LDL assays are validated in non-fasting states; your clinician specifies which method is required.
