Why vitamin D results vary between labs and what it means for monitoring

25-hydroxyvitamin D is the correct marker for vitamin D status, not 1,25-OH, the active hormone that fluctuates daily without reflecting stores. But 25-OH D results vary significantly between labs because immunoassay and LC-MS/MS mass spectrometry platforms produce different numbers for the same specimen. Reference ranges differ too: some labs flag deficiency below 20 ng/mL; others use 30 ng/mL. If you are tracking D levels longitudinally to monitor supplementation response, staying with the same lab and platform is essential. B12 and folate, causes of deficiency, and common confounders

Vitamin B12 deficiency is common in strict vegans, older adults whose gastric acid declines with age, and patients taking metformin long-term.
